Dental
Medicaid and private insurance are accepted. Medicare does not pay for dental services. Note: Please contact your insurance carrier directly if you have any question on your coverage. You are responsible for any non-covered services.
Struggling to Pay?
All patients – insured and uninsured alike – are welcome to complete an application for our Sliding Fee Discount Program. Patients who qualify are charged for services based on their income and family size. Patients should bring proof of income (for the last 30 days) to their first appointment. Income is due within 10 business days and can be emailed to patientsupportstaff@chcbh.org. By signing the Sliding Fee Discount Application you are confirming that all information shown is correct. And you understand the requirements for Proof of Income.

Income Guidelines
The same income guidelines and proof requirements still apply for the discount program.
Acceptable proof of income includes:
- 30 day pay stub
- Last year's taxes
- SSI/SSDI benefit letter
- Child support
- Retirement benefits
